Use one row per scenario: equipment, hazard, feared event, causes, consequences, and ratings before and after preventive measures.

What you will do

  1. Add one scenario for each hazard identified on a piece of equipment or in a zone.
  2. Rate initial severity and probability, describe preventive measures, and then rate the scenario again after those measures.

Pitfall to avoid. Calibrate the risk matrix on the Metrics and Matrices page before rating many scenarios. Re-rating later takes longer than configuring the scale correctly first.
